PARENTAL CONSENT FORM

Accompanied:

I, [Parent’s name], [passport number], am the parent/guardian of [Name of child], [passport number], who will be travelling to the UK as part of an Approved Destination Scheme ADS tour group.

The child will be accompanied by [Name of adult], [passport number], who will be responsible for their support and welfare throughout the trip. The relationship of [Name of adult] to [Name of child] is [relationship, eg grandparent/aunt/family friend].

I consent to the issue of an ADS - Visit visa to [Name of child], that the visa will be endorsed with the passport number of [Name of adult] and that the visa will not be valid for travel unless this adult is accompanying them.

I understand that if the accompanying adult is no longer travelling with the tour group I will need to apply for a new visa for [Name of child] and submit a new consent letter naming the new accompanying adult.

Unaccompanied:

I, [Parent’s name], [passport number], am the parent of [Name of child], [passport number], who will be travelling to the UK as part of an Approved Destination Scheme tour group. They will be accompanied throughout the trip and during entry and departure from the UK by a tour guide employed by a UKVI approved ADS Agent.

I consent to the issue of an ADS - Visit visa to [Name of child] and that this visa will not be endorsed with the details of an accompanying adult.
